Overview

Facilitates, Coordinates, Provides PA&I functions.

EDUCATION, EXPERIENCE, TRAINING

  1. Registered Nurse.
  2. -Knowledge of and ability to use Excel, Word, web-based programs and Outlook and learn new programs
  3. -2 yrs. acute hospital experience
  4. -Medical terminology
  5. -Ability to work independently with little supervision
  6. -Ability to multitask and prioritize
  7. -Strong communication skills and the ability to effectively work with a diverse group of individuals.
  8. -Ability to assess and analyze data and compile reports with guidance from the ICP.
  9. Critical care experience required.

     

    Responsibilities:

    Review all admissions for focused criteria (current and any future core measures or other initiatives)

    Concurrent review for all admission, continued stay and discharge indicators

    Follow core measure patients after DC to prevent readmission.

    Identifies failures in the DC system and recommends solutions to try and prevent readmissions.

    Communicates with physicians for improved documentation to demonstrate best practice in the care of these patients

    Works directly with PACE Team leaders to present identified process improvements utilizing aggregate data

    Assists in preparing team leaders to present findings at IOP committee timely

    Provides ongoing education to staff and physicians related to best practice

    Participates in appropriate team activities

    Utilizes effective critical thinking skills; organization, prioritization, planning, personnel management, operational efficiency, decision-making, and teaching

    Participates in concurrent and retrospective audits and data abstraction of assigned patients and records.  

    Participates in concurrent and retrospective audits and data abstraction of CATH/PCI/TAVR patients and records.  

    Identifies failures in the CATH/PCI system for improvement. Recommends solutions.

    Completes data abstraction timely

    Presents data to medical staff committees for improvement and education

    Review all fall outs for lack of compliance and educate as appropriate

    Become expert for best practice, documentation and abstraction

    Acts as a liaison and resource with hospital staff providing education on Quality Initiatives
