This position is responsible for assisting in the implementation of an effective facility-wide Infection Prevention program in keeping with Centers for Disease Control (CDC), Joint Commission, OSHA, Title 22, Center for Medicare and Medicaid Services (CMS), California Department of Public Health (CDPH), and other regulatory agencies. JOB SUMMARY: The Case Manager uses InterQual criteria to evaluate the appropriateness of admissions, concurrent stay and readiness for discharge. She/he assures correct level of care: critical, telemetry, acute, observation, for all patients. The Case Manager provides an efficient, coordinated approach to clinical management, with an emphasis on discharge planning, care coordination and utilization management activities for select patient populations across the continuum of programs and services at GMC. The essential functions of the Case Manager require constant collaboration with the Social Workers, Nursing staff, Financial Counselors, Medical Staff to coordinate the care of patients in a proactive manner.

Applies appropriate theory as a basis for decisions regarding nursing practice. Perform initial physiological and psychosocial needs assessment on all patients within 30 minutes of admissions and shift assessment within (2) hours of coming on duty. Communicates appropriate data to the physician and other patient care team members. Collaborates with other discipline to plan and coordinate care.
