Overview

The Sub-Acute Director is responsible for the supervision of the nursing staff as well as
oversight of the Unit Social Worker, Activities Leader and Program Secretary. This
position administers the nursing program in Hospital in order to maintain standards of
patient care. Recommends establishment or revision of policies and develops
standards of performance. Interprets policies and objectives of Hospital nursing
service to staff and community groups. Promotes good working relations with
community groups. Promotes a good working relationship with community agencies
and with other Hospital departments. Performs personnel management functions, such
as establishing personnel qualification requirements, drafting procedure manuals,
initiating in-service programs and installing record and reporting systems. Appoints
professional and auxiliary nursing staff. Initiates studies to evaluate effectiveness of
nursing services in relation to its objectives and cost. Participates in community health
programs.

Responsibilities

1. Collaborates with the case management, business development team
in assigning and evaluating patient admissions to Subacute.
2. Assures that an accurate assessment of resident care needs is
completed and documented for each resident within 14 days of
admission to the Unit.
3. Assures that and individualized inter-disciplinary plan of care
containing measurable and time limited objectives is established for
each resident and reviewed at least quarterly or as a change in
resident condition warrants.
4. Consistently encourages the SNF/Subacute staff to anticipate
potential health and psychosocial deficits and in identifying nursing
actions to minimize or prevent those deficits.
5. Ensures that infection control policies and procedures are adhered to,
especially those dealing with Universal Precautions and the handling
of hazardous wastes.
6. Assures that resident response to nursing or other professional
treatment interventions is evaluated and documented on all residents.
7. Directs and supervises nursing care on a 24 hour basis.
8. Continuously evaluates the quality of nursing care delivered on the
Unit.
9. Maintains a safe therapeutic environment.
10. Consistently demonstrates the ability to work effectively with Unit staff,
physicians, families, Hospital administrative team, outside resources,
ancillary service personnel and GEMCH personnel.
11. Makes resident care rounds daily.
12. Plans and conducts resident care conferences.
13. Develops implements and assures staff compliance with Hospital
quality & performance improvement plan

14. Interprets and directs implementation of policies and procedures.
15. Provides for adequate orientation of all personnel according to Title 22
standards and OBRA guidelines.
16. Assures through regular observation and evaluation that nursing staff
on the Unit are proficient in the delivery of resident care and are
competent to perform all necessary treatments and procedures.
17. Demonstrates concern for residents’ welfare by rendering immediate
assistance to residents in need.
18. Promotes, maintains and helps establish an environment that offers
residents, family members, significant-others, and the community,
courteous, fair and impartial service.
19. Consistency demonstrates effectiveness in directing and coordinating
the activities of the Unit in accordance with Hospital and departmental
policies.
20. Performs such other functions as are identified by the hospital
COO/Administrator and CNO

Qualifications

Must have strong leadership skills
Must have strong communication skills
Current valid licensure to practice as a Registered Nurse inCalifornia
Current Basic Life Support Certification
Minimum of three years nursing experience required, one of whichhas been in Supervision

Experience caring for patients with technology dependency (MediCal requirement)
Previous SNF experience
Must have knowledge, understanding & familiarity with DHS,federal, state and local regulation standards. 

Bachelor’s Degree in nursing preferred
