The Manager reviews, assesses, identifies high risk resource consumptive cases, coordinates the primary care team staffing in Case Management, Discharge Planning, and other departmental activities. Continuously evaluates outcomes identifying alternatives preventing unnecessary costs and services while ensuring quality care, patient satisfaction, and timely discharges. Supervises the Case Managers, accompanies Physician Advisor on high-risk patient rounds. Identifies and makes recommendations to Director, Physician Advisor and staff on complex cases to ensure quality of care and appropriate utilization.
EDUCATION, EXPERIENCE, TRAINING
RN/LVN License in the State of California required
Certified Case Manager preferred.
