The Case Mix Coordinator is responsible for the tracking of Medicare patient admissions and discharges, including the timely completion of Admission and Discharge of Inpatient Rehabilitation Facility- Patient Assessment Instrument ( IRF-PAI). Also responsible for data entry of IRF-PAI , encoding and timely transmission to CMS. The Case Mix Coordinator must work closely with the Admitting Rehabilitation Physician to ensure accurate and complete documentation; medical records Coders to ensure accuracy of primary diagnosis and comorbidity coding; therapy, nursing, admissions coordinators and social workers to ensure accurate and timely functional data recording; and billing office staff to ensure accurate completion of the patient bill with appropriate CMG code. Other duties include pre-screening patients, conducting continued stay reviews and team conferences; interfacing with referring physicians, discharge planners and case managers. Participates in marketing and community educationregarding Acute Rehab. Program.
