  • Overview

    The Post-Acute Care Liaison works under the direct supervision of the Department Director, assisting and coordinating referrals and admissions for the hospital. Responsible for establishing and maintaining relationships with all potential and existing referral sources, including physicians, hospitals and post- acute administrators, discharge planners, nurses, social workers and community service agencies. Develops and nurtures positive relationships with potential patients/families and internal and external referral sources, promotes awareness of Post-Acute Care services in the community.  Always assists the department and staff functionally in the provision of high quality patient-family centered, in the most cost effective manner. Provide customer service for patients and family members needing assistance.

    • Assists the Director of Patient Care Services in providing leadership in identifying opportunities for additional or improved services to address customer needs.
    • Collaborates with the Post-Acute Administrators, Director of Nursing, Director of Admissions, and Case Managers, and Social Services for effective admission and discharge processes resulting in higher patient satisfaction.
    • Assists case managers, discharge planners and social workers on placement of difficult and homeless patients.
    • Provides a smooth transition for patients and families by coordinating care with case managers, nursing staff, social workers, physicians, discharge planners and vendors.
    • Participates in ensuring key metrics and department goals are met regarding patient flow from hospital to skilled nursing homes, assisted living, subacute and long term acute (i.e. length of stay and readmission rate).
    • Helps build and monitor community, customer payer and patient perceptions of the Hospital as a high quality provider of services.
    • Establishes and maintains positive working relationships with current and potential referral and payer source.
    • Apprises other support staff of daily items that require attention/action to ensure an up-to-date status on projects and/or events.
    • Provides periodic in-service training and education to physicians and staff on insurance verification, admission criteria and ensuring authorization throughout post-acute care hospitalization.
    • Provides in-service education of homeless patient resources.
    • Provides in-service events for skilled nursing, assisted living, and other post-acute providers.
    • Develops and maintains a database of key contacts at skilled nursing facilities, assisted living communities, and other post-acute providers.

    Qualifications

    • Must have a minimum of a bachelor's degree.
    • Must have at least five (5) years healthcare experience in either hospital, managed care organization or post-acute care organization.
    • Knowledge of health insurance and coverage requirements.
    • Knowledge of the nearby hospital market place with established contacts (preferred).
    • Strong level of organizational skills.
    • Knowledge of medical terminology (preferred).
    • Proficient in Word, Excel, and Powerpoint.
