  • Responsibilities

    • Demonstrates proficiency in the operation of the computer and related software components. This includes basic trouble shooting skills and proficiency in the operation of the MDS software and all related components.
    • Demonstrates ability to conduct and coordinate the initiation and completion of each resident’s assessments in a timely and accurate manner
    • Maintains a tracking calendar to assure all MDS are performed and completed in accordance with State and Federal Regulation.
    • Perform physical assessment and other data collection techniques to assure that all required assessments of the resident are completed accurately thoroughly and timely.
    • Coordinate all disciplines in assessing the residents – educating IDT members regarding data collection and reporting for MDS and communicating MDS assessment schedule to ID Team. Reports any noncompliance from ID Team Members for RAI process completion to DON for follow up.
    • Select the appropriate type of assessment including the initiation of significant changes of condition
    • Demonstrate ability to input all MDS data into the computer within the required timeframes
    • Demonstrates ability to complete triggered care areas (RAPS/CATS) and formulate comprehensive care plan based upon assessment findings and trigger areas.
    • Demonstrates knowledge of the current RAI process and guidelines
    • Demonstrate ability to transmit data to internal or external database (MCDN/State).
    • Demonstrates ability to interpret and analyze transmission report, facility quality indicators and MDS Online Reports.
    • Demonstrates ability to create back-up files and store electronic data related to the MDS (as appropriate – working with Information System Director as needed)
    • Demonstrates the ability to educate staff and IDT members regarding RAI and Care Planning process.
    • Demonstrates ability to correct MDS assessments according to State and Federal requirements – including modification and inactivation of submitted MDS assessments
    • Create and provide/coordinate educational opportunities and competency skills for pediatric subacute related care for all levels of nursing on peds subacute unit. Competencies performed in selected topics will be required by staff at least annually and more frequently if the need is identified or any process is changed. Inservices for MDS/RAI process as needed.
    • Create and provide/coordinate educational opportunities for all staff with the initiation of any new policy, procedure, or new equipment or supply usage.
    • Coordinate with DON and Clinical Managers to ensure all care plans (short and long term) are consistently updated, current and relevant to resident’s individualized plan of care as determined by IDT recommendations and resident preferences/needs

    Qualifications

    RN license and PALS and BCLS required.  DSD certificate required.  One year subacute care RN experience.  Charge or supervisory experience preferred.
