Additional responsibilities include directing the unlicensed employee’s activity, synthesizing patient information, assessment of patient’s, planning of care/intervention, teaching of patients/family, implementation of treatment plan/MD orders, and documentation and evaluation of patient care. The Case Manager Registered Nurse is primarily responsible for the preparation, recording and tracking of initial and retrospective payments for hospital services. Answers, directs and records departmental phone calls from a variety of sources (Insurance companies, Skilled Nursing Facilities, Home Health Agencies, Durable Medical Equipment, other health care providers, etc.) understanding priorities for the delivery of quality patient care. Coordinates with Discharge Planners for continuum of patient care, for example, durable medical equipment (DME) and Levels of Care.
