Coordinates with Discharge Planners for continuum of patient care, for example, durable medical equipment (DME) and Levels of Care.

The Ambulatory Surgery Center RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the therapeutic environments, treatment and modalities and interdisciplinary collaboration.  Applies appropriate theory as a basis for decisions regarding nursing practice.  Perform initial physiological and psychosocial needs assessment on all patients within 30 minutes of admissions and shift assessment within (2) hours of coming on duty.  Communicates appropriate data to the physician and other patient care team members.  Collaborates with other discipline to plan and coordinate care.  

In collaboration with the nursing and physician staff, the emergency technician assists in providing timely and appropriate patient-family centered care for all patients in the Emergency Department.   In order to assist the Emergency Department Team, this position requires an understanding of care and service needs of critically ill through acutely ill pediatric through geriatric population.              This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center. The unit secretary / monitor technician through communication, requisitioning and management of supplies and equipment, coordinates the activities of the nursing staff, the patient, the physician, other hospital departments and visitors. He/she performs general receptionist and clerical duties in order to facilitate efficient functioning of the nursing unit. Is skilled in reading and interpreting cardiac monitors and arrhythmias.

The Registered Nurse provides care, treatment, counseling and health education to the hospitalized patient.  The RN is responsible for planning and coordination of care across the continuum.  This planning and coordination includes meeting and exceeding compliance with core measures, contributes to reduction in LOS, active participation with all aspects of quality, safety and performance improvement activities, collaboration with all the members of the multidisciplinary team in providing outstanding customer service to patients, family members, physicians and the public.  Meets and exceeds the Department of Health Service (DHS), The Joint Commission (TJC) and other regulatory requirements.

The Staff Speech Pathologist organizes and coordinates speech pathology treatment programs.  He/she participates in direct patient evaluation and treatment to facilitate and/or restore speech, language, cognitive and swallowing functions and collaborates with interdisciplinary treatment team.  EDUCATION, EXPERIENCE, TRAINING   - Master’s Degree in Speech Pathology. - Valid California Speech Pathology License (or eligible). - Certificate of Clinical Competence (CCC) from American Speech Language and Hearing Association (ASHA), or eligible. - Current BLS, American Heart Association   Preferred if possible: FEES Certification Experience with Modified Barium Swallow  Evaluation and treatment plan for all speech and swallow patients in the acute, rehab and Outpatient settings.  May include Modified Barium Swallow evaluations, cognitve training, swallow training

The Charge Nurse is responsible and accountable for the quality of staff and functions during their shift.  Directly supervises and communications with all staff along with the Director of the department. Assists the Department Director in the daily operation of the unit, directing, supervising and evaluating personnel, ensuring the proper use of time, equipment and staff. Ensure the efficient functions of the department. Performs direct patient care, only as needed, within the scope of practice. Continually assess care and direct staff to make immediate changes in the plan of care. This position is responsible for providing patient care in a safe and effective manner.  Responsibilities include directing PCA activity, synthesizing patient information, assessment of patients, planning of care/intervention, teaching of patients/family, implementation of treatment plan, MD orders, documentation and evaluation.  Integration of patient care standards are incorporated into all clinical activities.

The Insurance Verifier provides administrative support and performs general office functions in the Outpatient Bariatric Department. Completes insurance verification and coordinates patients’ clearances with their doctors.

As a member of the outpatient Bariatric health care team the Medical Assistant will work directly with the clinic team under the direct supervision of the Bariatric Clinic Coordinator, the Registered Nurse Bariatric Program Manager and/or the physician to support and ensure efficient operation of the Bariatric Clinic.

The HIM Technician is an experienced HIM clerk who has a minimum of 2 years’ experience as a clerk.
