JOB SUMMARY: Facilitates, directs, coordinates, and provides guidance and education to the Administrative team, physicians, department heads and other employees in UR/QI and related areas.  

The Unit Coordinator provides clerical duties for the Unit under the direction of a Registered Nurse or Physician.

The Nursing Administrative Supervisor will act as an administrative representative on assigned shifts, be responsible for the effective management of patient care on assigned shifts, and contributes toward attainment of departmental and hospital goals and objectives.

Overview Under the general supervision of the case management Director acts as a patient advocate/case manager to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Looks for opportunities to reduce cost while ensuring the highest quality of care is maintained. Applies review criteria to determine medical necessity for admission and continued stay. Provides clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost-effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care.  Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care.

The Registered Nurse in the Operating Room provides safe and effective care of the perioperative patient. The OR RN is responsible for assessing the patient, planning and implementing care specific to each patient's needs. The OR RN coordinates the care of patients with the other members of the OR team, consisting of the surgeon, anesthesiologist, and surgical technician adhering to the Standards of Care of the Association of Operating Room Nurses (AORN), the regulatory requirements of The Joint Commission (TJC), the Department of Health Services (DHS), and Title 22 from the California Code of Regulations.

Surgical tech I for part time 3 days a week Performs technical skills and provides assistance to surgeons, anesthesia personnel, registered nurses, and other professionals in the operating room. Performs similar duties in other areas of the hospital as assigned. Independently scrubs on all surgical procedures. Demonstrates ability to problem solve in relation to the procedure being performed. Applies basic knowledge of anatomy and physiology, medical terminology, microbiology, and pharmacology for optimum surgical patient care. Demonstrates knowledge and practice of patient care concepts. Applies the principles of asepsis during surgical procedures. Qualifications: Two years Surgical Tech experience preferred. Completion of approved Surgical Tech course requires. Current BLS certification required.

Maintains records; performs and records instrument maintenance; troubleshooting instrument and sample problems; regularly utilizes the lab computer for various aspects of daily work; verifies completion of work using the computer. Able to work independently in a generalist capacity while adhering to standard lab practices.

The Lead Clinical Laboratory Scientist, Evening shift, performs laboratory procedure on specimens from inpatient and outpatients and assist the Coordinator in the daily operations of the Laboratory. Performs intrument responsibilities and all basic LIS functions. Assist in maintaining adequate supplies by communicating shortages and preparing reagents as needed. Acts under the direction of Lab Director, Coordinators and/ or Pathologists in other necesssary activities consistent with training, education, and professional ethics.

The Certified Nursing Assistant (CNA), is a member of the health care team who assists in the delivery of total patient care, under the direction of a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).

JOB SUMMARY: The GeroPsych Social worker provides general casework services to assigned patients and their families. They are to help people with mental impairments realize their full potential. To deliver and manage excellent services and systems of care for persons with serious mental illness. 

The Long Term Care Staff Development Educator is responsible for planning, conducting, and coordinating educational programs for the development and improvement of necessary skills and knowledge for all Long Term Care (LTC) personnel.  The Staff Development Educator ensures orientation, certification, and inservice programs are conducted according to facility policy and procedure and in compliance with state and federal regulations.  The Staff Development Educator will conduct inservices as mandated by state and federal regulations and that address the needs of the facility as determined by the facility's Quality Improvement Program, through completion of clinical rounds as identified by the Director of Long Term Care/designee, or as identified during a Licensing and Certification visit.

Regularly performs laboratory procedures in the assigned scientific sections of the Clinical Lab and/or Emergency Department.  Performs necessary quality control procedures and maintains records. Troubleshoots instrument problems. Performs and records instrumentation. Performs other clinical and clerical duties as needed. Regularly utilizes the laboratory computer for various aspects of daily work. Verifies completion of work by various computer programs. Communicates with nursing and other hospital personnel, regarding laboratory procedures, results and overdue tests. Runs and completes all testing that is permitted under the licensure by the State of CA. Qualifications: CA licensed Medical Laboratory Technician and AA degree. Medical Laboratory Technician Internship preferred.

Prepares and serves meals for patients, employees, visitors. Responsible for meals in Catering Center as needed; overseeing kitchen operations.

 The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at AHMC Seton Medical Center, American Psychiatric Mental Health Nursing Scope and Standards of Practice, and American Nurse Practice Act. Staff Nurse ll’s are capable of independently performing the full range of clinical nursing services involving patient care. May supervise the work of professional and paraprofessional staff of the assigned unit.  

In collaboration with the Patient Experience staff, will perform purposeful daily rounding on patients and families in the facility to assess patient needs and collaborate with nursing and other staff to ensure that needs are met. The staff member will demonstrate professional courtesy at all times, andwill role-model excellent verbal, and non-verbal communication skills. Monitors the admissions, continued stay, and discharge of patients following pre-­established criteria.   Assures that patients meet InterQual criteria from admission to discharge including appropriateness of level of care.   Conducts interdisciplinary care management rounds.  Ensures collaboration between multidisciplinary healthcare team members, primary physician, community agencies, HMOs/PPOs, CCS, etc., whose services may be required and/or related to the care needs of the patient after hospital discharge.  Monitors nursing and medical plans of care/discharge plans and provides appropriate interventions to assure care is appropriate, coordinated and that avoidable patient days are addressed effectively through education, consultation, and counseling as needed.  Ensure patient centered discharge planning and assessment by communicating the appropriate discharge information and instructions to the primary care giver and primary physician and/or follow-up care agency.   Assures patients are transferred to appropriate approved facilities when required.   This position requires providing service to an ill through rehabilitating neonatal through geriatric patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served.   This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center.
