The Physical Therapist provides physical therapy skilled intervention to patients in an acute care hospital setting as a needed basis. This role involves evaluating patients, developing, and implementing indidualized treatment plans, and collaborating with the interdisciplinary healthcare team to promote optimal functional outcomes and safe discharge planning.
The key responsibilites for the physical therapist includes performing comprehensive physical therapy evaluations for patients with various medical condictions, including post surgical, neurological, orthopedic, and general medical diagnoses. Develop and implement individualized treatment plans based on clinical findings and patients goals. Provide therapeutic interventions, including therapeutic exercises, therapeutic activities, balance/coordination training and gait training. Educate patients, families, and caregivers on treatment plans, home exercises, and safety measures. Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care and contribute to discharge planning. Document accurately all evaluations, progress notes, and discharge summaries in accordance with hospital policies, and regulatory standards in a timely manner. Maintain compliance with hospital policies, safe regulations, infection control protocols, and professional standards of practice. Provide coverage at least one weekend a month or more weekdays as the need arises, including holidays if required.
Licensed CA State Board Physical Therapist (PT). Current BLS certification.
