The RN Cardiovascular Manager is a registered nurse who is accountable for effective management, coordination of care, development of patient education, data collection and application, and care management oversight for our cardiac patient population. Patients within this scope of care include those who are being evaluated for advanced cardiovascular disease and cardiovascular procedures and treatments. The Cardiovascular Clinical Coordinator guides inpatient and outpatient care in partnership with the critical care service line, interventional cardiologists, the cardiothoracic surgical team, electrophysiologists, advanced practitioners, and clinical staff. Alongside this multidisciplinary team, the Cardiovascular Clinical Coordinator delivers evidence-based care with a patient and family-centered approach and strives to meet or exceed the standards of quality care set forth by AHMC Anaheim Regional Medical Center Critical Care Service Line.
The RN Cardiovascular Clinical Coordinator has a developed knowledge base and nursing expertise in cardiovascular diseases and treatment modalities, including invasive and surgical interventions (TAVR, CABG). The RN Cardiovascular Clinical Coordinator will:
Coordinate clinical management of inpatient cardiology patients, participate in direct patient care with emphasis on patient education, provide key aspects of cardiovascular patient education.
Triage telephone calls and respond to patients and families and other members of the healthcare teams questions that are within a registered nurse’s scope of practice. Develop nursing care plans to ensure standards of care are achieved and promote long-term heart health. Responsible for the coordination of care for TAVR and CABG patients.
Participate in data collection / processing: Perform accurate and timely data collection, analysis and reporting to support national registries (National Cardiovascular Data Registry - NCDR, Cath PCI registry, the Society for Thoracic Surgeons and the American College of Cardiology - STS/ACC TVT Registry), process improvement, and quality initiatives. Manage and interpret all reporting data and audits for compliance measures.
Facilitate quality improvement and promote clinical workflow. Proactively review patient charts and schedules to identify and resolve potential conflicts. Verify appropriate titration and electronic documentation of ordered IV medications. Ensure appropriate resources are available to support patient care and procedures. Adapt to new roles and responsibilities as required to fulfill team and organizational goals.
