The Social Service Designee is responsible for assisting in meeting the psychosocial needs of residents and families in a skilled nursing facility (SNF) environment. Provides consultations and casework and assists residents and families in obtaining resources appropriate for their identified needs. Coordination of discharge planning process for residents includes: referrals to community resources and ordering of durable medical equipment. 

Overview Under the general supervision of the case management Director acts as a patient advocate/case manager to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Looks for opportunities to reduce cost while ensuring the highest quality of care is maintained. Applies review criteria to determine medical necessity for admission and continued stay. Provides clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost-effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care.  Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care.

The Registered Nurse in the Operating Room provides safe and effective care of the perioperative patient. The OR RN is responsible for assessing the patient, planning and implementing care specific to each patient's needs. The OR RN coordinates the care of patients with the other members of the OR team, consisting of the surgeon, anesthesiologist, and surgical technician adhering to the Standards of Care of the Association of Operating Room Nurses (AORN), the regulatory requirements of The Joint Commission (TJC), the Department of Health Services (DHS), and Title 22 from the California Code of Regulations.

- Under the supervision of the Nuclear Medicine Physician, prepares, calibrates, logs, and administers radioactive isotopes to patients undergoing a variety of nuclear medicine procedures. Operates Gamma Camera imaging equipment, computers, and other isotope sensitive equipment to obtain a visual representation or measurement or isotope concentrations in specific body areas/organs. - This position requires providing care/service to a critically ill through rehabilitating pediatric through geriatric patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served. - This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center. Responsible for ordering the special surgical supplies and keeps the inventory as needed and appropriate par level. Duties and responsibilities demand comprehensive knowledge of surgical supplies and work closely with clinical manager. The Relief Stationary Engineer is responsible for providing a full range of maintenance and repair and administrative support to Seton Medical Center. Provides excellent customer service.

Under general supervision and guidelines, obtains and verifies financial sponsorship, patient demographics, and clinical information. Utilizes skills and knowledge of department / hospital policies and procedures to register patients, verify insurance benefits, and collect payments. Works in a team environment and must be able to work independently in all areas of registration, including E.R. registration, insurance verification, pre-admission, Out-patient registration, and cashier with minimal supervision. The Radiologic Technologist ( II) is responsible for performing a variety of radiographic, fluoroscopic and specialized procedures at a technical level not requiring constant supervision, under the general direction of a radiologist and Radiology Administration.  Collaborate with department and multidisciplinary team to ensure quality patient care and department service.

This position is responsible for providing patient care in a safe and effective manner. Responsibilities include directing PCA activity, synthesizing patient information, assessment of patients, planning of care/intervention, teaching of patients/family, implementation of treatment plan, MD orders, documentation and evaluation. Integration of patient care standards are incorporated into all clinical activities.

The Social Services Designee is the primary purpose of your job position is to plan, develop, organize, implement, and direct in according with current and existing Federal, State, and Local standards, as well as our established policies and procedures to ensure that the emotional and social needs of the resident are met and maintained on an individual basis.

The Monitor Tech/Unit Secretary, reporting to the Nursing Unit Director, with assignment and direction from the Charge Nurse or Registered Nurse, is responsible for assisting in the planning, implementing and evaluating the activities occurring in the nursing station by performance of clerical/ receptionist &/or Cardiac Monitoring duties.  This position is responsible for maintaining the physical environment of the nursing unit. The MT/US supports the hospital mission and vision with emphasis on customer relations. The MT/US utilizes knowledge of patient’s age, spiritual and cultural diversity into unit communication & organization. The US is a non-licensed position whose training & skills enables him/her to interpret basic cardiac rhythms, recognize & alert the RN to significant rhythm changes or common dysrhythmias. The position contributes to the provision of safety and quality care that result in positive outcomes in patient care. 

Lead LVN is responsible for the coordination of assessment, data collection, and submission of the Minimum Data Set ( MDS) & Payroll Based Journal ( PBJ). The position will be responsible for MDS, PBJ, and conversion & maintenance of paper medical records to electronic medical records. Assist with staffing of unit, staff evaluation, education of staff, other various duties assigned by the unit director. 

This position is responsible for providing diagnostic and therapeutic procedures as defined by departmental policies and procedures for neonatal through geriatric patients. Responsibilities include general respiratory patient care, emergency care, critical care, patient assessment, and patient plans of care, patient/family education and patient/family discharge instruction. This position also requires utilization of advanced practitioner skills to intervene in complex patient care situations. The Respiratory Care Practitioner II assists in the development of policies and procedures, patient protocols, education/coaching of nursing, support personnel, patients, and caregivers and participates in multidisciplinary patient care conferences and performance improvement activities.

Able to work in all basic functions of a CLS in the main lab, blood bank and microbiology. Performs quality control procedures for routine and stat testing of patient samples. Maintains records; performs and records instrument maintenance; troubleshooting instrument and sample problems; regularly utilizes the lab computer for various aspects of daily work; verifies completion of work using the computer. Able to work independently in a generalist capacity while adhering to standard lab practices.

Performs technical skills and provides assistance to surgeons, anesthesia personnel, registered nurses, and other professionals in the operating room. Performs similar duties in other areas of the hospital as assigned. Independently scrubs on all surgical procedures. Demonstrates ability to problem solve in relation to the procedure being performed. Applies basic knowledge of anatomy and physiology, medical terminology, microbiology, and pharmacology for optimum surgical patient care. Demonstrates knowledge and practice of patient care concepts. Applies the principles of asepsis during surgical procedures.
