JOB SUMMARY: The GeroPsych Social worker provides general casework services to assigned patients and their families. They are to help people with mental impairments realize their full potential. To deliver and manage excellent services and systems of care for persons with serious mental illness. 

The Long Term Care Staff Development Educator is responsible for planning, conducting, and coordinating educational programs for the development and improvement of necessary skills and knowledge for all Long Term Care (LTC) personnel.  The Staff Development Educator ensures orientation, certification, and inservice programs are conducted according to facility policy and procedure and in compliance with state and federal regulations.  The Staff Development Educator will conduct inservices as mandated by state and federal regulations and that address the needs of the facility as determined by the facility's Quality Improvement Program, through completion of clinical rounds as identified by the Director of Long Term Care/designee, or as identified during a Licensing and Certification visit.

Regularly performs laboratory procedures in the assigned scientific sections of the Clinical Lab and/or Emergency Department.  Performs necessary quality control procedures and maintains records. Troubleshoots instrument problems. Performs and records instrumentation. Performs other clinical and clerical duties as needed. Regularly utilizes the laboratory computer for various aspects of daily work. Verifies completion of work by various computer programs. Communicates with nursing and other hospital personnel, regarding laboratory procedures, results and overdue tests. Runs and completes all testing that is permitted under the licensure by the State of CA. Qualifications: CA licensed Medical Laboratory Technician and AA degree. Medical Laboratory Technician Internship preferred.

Prepares and serves meals for patients, employees, visitors. Responsible for meals in Catering Center as needed; overseeing kitchen operations. 

The Licensed Vocational Nurse (LVN) provides appropriate nursing care under the direction of the Registered Nurse (RN) in compliance with the standards of care at Seton Medical Center, AHMC and the LVN Nurse Practice Act; demonstrates respect for the individual and maintains the dignity of the patient and family unit.

Staff Nurse ll’s are capable of independently performing the full range of clinical nursing services involving patient care. May supervise the work of professional and paraprofessional staff of the assigned unit.  

In collaboration with the Patient Experience staff, will perform purposeful daily rounding on patients and families in the facility to assess patient needs and collaborate with nursing and other staff to ensure that needs are met. The staff member will demonstrate professional courtesy at all times, andwill role-model excellent verbal, and non-verbal communication skills. The employee will respond to basic non-clinical concerns and provide appropriate referral for further assistance when needed. - Completes daily rounds of inpatients and periodic rounds of outpatients/Emergency Department patients. The Pharmacy Technician will help to provide high quality and cost-effective pharmaceutical service to support the patient care needs. Processes new medication orders with accurate interpretation of the order, labeling of the medication and selection of the medication dosage and quantity needed until the next cart exchange with 99% accuracy.

Monitors the admissions, continued stay, and discharge of patients following pre-­established criteria.   Assures that patients meet InterQual criteria from admission to discharge including appropriateness of level of care.   Conducts interdisciplinary care management rounds.  Ensures collaboration between multidisciplinary healthcare team members, primary physician, community agencies, HMOs/PPOs, CCS, etc., whose services may be required and/or related to the care needs of the patient after hospital discharge.  Monitors nursing and medical plans of care/discharge plans and provides appropriate interventions to assure care is appropriate, coordinated and that avoidable patient days are addressed effectively through education, consultation, and counseling as needed.  Ensure patient centered discharge planning and assessment by communicating the appropriate discharge information and instructions to the primary care giver and primary physician and/or follow-up care agency.   Assures patients are transferred to appropriate approved facilities when required.   This position requires providing service to an ill through rehabilitating neonatal through geriatric patient population in a manner that demonstrates an understanding of the functional/developmental age of the individual served. - Performs admission and continued stay review utilizing criteria approved by the medical staff to ensure that patients meets Severity of illness/intensity of Service criteria. - Confers with the attending/consulting physician(s) as appropriate when the medical necessity for admission or continued stay in not clear. - Consults with the Physician Advisor when the admission or continued stay does not meet criteria, care is not being provided timely or does not meet the community standard of care. - Provides clinical review information to external review entities or insurance companies to ensure authorization for admission and continued stay is obtained. - Accurately completes the MediCal Treatment Authorization Request (TAR) in detail to ensure payment for hospital services. - Refers medically complex patients under the age of 65 to the MediCal Case Management program. - Facilitates transfer of patients to other acute care facilities as required either due to third party payer requirements or county indigent program. - Identifies potentially avoidable days, delays in service, over utilization or quality of care issues and completes reports as required. - Performs the Case Management Admission Assessment within two working days of admission. - With the concurrence of the patient, family and physician develops a plan for post discharge care. - Refers appropriate patients to Social Services for psychosocial intervention, Advance Directive or end of life education. - Accurately documents the case management process in the medical record on the Discharge Planning Assessment form N-245. - Identifies the responsibilities and involvement of the Inter-disciplinary team members in discharge planning activities on an ongoing basis. - Participates in Interdisciplinary Care rounds/conferences to facilitate coordination of care, goal setting, and developing strategies to facilitate the discharge planning process. - Communicates the final discharge date and plan with the patient and family to ensure that they are informed as required by law and documents such notification in the Plan section of the Discharge Planning Assessment form (N-245). - Provides accurate information and completes referrals as appropriate to implement the discharge plan including but not limited to Home Health Services, Hospice, Skilled Nursing Facilities, Durable Medical Supplies, and other community resources. - Maintain confidentiality as required by HIPPA and only provides information relating to payment, hospital operations or continuity of care. - Provides “hand off” information to the receiving Case Manager to ensure a safe, smooth transition to other nursing units. - Refers situations requiring immediate intervention to the Director of Case Management, Risk Management, Director of Quality Management and the Vice President of Medical Affairs. - Participates in committee meetings, patient care conferences other activities as assigned. - Participates in department Performance Improvement activities. - Participates in the orientation of new employees or cross training other case managers as needed. - Maintains accurate records and statistics of case management activities, as required. - Demonstrates a continuing effort to improve the quality of case management performance through on-going education. - Incorporates the core values; dignity, collaboration, justice, stewardship and excellence into daily performance. - Performs other related duties as assigned or requested.

The position of Education Coordinator reports to the Chief Nursing Officer. The mission of education and training is to promote excellence through the continuing education and support of patient care and ancillary staff. Performs the essential functions of mission integration, team work, time management, communication, interpersonal relationships, organizational contribution, professional manner and appearance, professional practice and generic education planning and implementation in collaboration with the leadership team.  

 Position Summary                 The Patient Account Specialist review and completes charity applications for state and local funding. Assist patients and physician’s office with billing questions in a cheerful approach. Review accounts and provide needed information as requested by the Central Business Office for proper reimbursement. Assist the Patient Access Director in the daily operations at the facility level. Approaches all other duties as assigned with a positive and team sprit. The position will also be expected to support all organizational expectations including, but not limited to Customer Service, Patient’s Rights, Confidentiality of Information, Environment of Care and AHMC initiatives.    

The Clinical Manager is responsible for providing direction and clinical supervision of patient care for specified unit, patient population and assigned staff. The Clinical Manager is a professional caregiver with requisite clinical and supervisory experience, which promotes an atmosphere that is responsive to the needs of patients, nursing and medical staff, and will assume oversight of the unit as necessary. The Clinical Manager will provide education and training to improve the clinical competencies of the staff. The Clinical Manager is knowledgeable about and promotes the philosophy, goals and objectives of the department.

POSITION SUMMARY Under the administrative direction of the Chief Executive Officer (CEO), the Chief Financial Officer (CFO) is the highest-level financial executive. The Chief Financial Officer (CFO) has complete oversight of all fiscal functions within Monterey Park Hospital (MPH) including the development, interpretation, coordination, and administration of the hospital’s policies on finance, accounting, insurance, financial/accounting systems, internal controls, budgeting, and auditing. The CFO oversees the planning and direction of all finances, and formulates long-range goals and plans for expansion. Develops and establishes operating policies consistent with overall objectives and ensures their adequate execution. The CFO manages, directs, plans and coordinates the functions and activities of assigned areas; serves as part of the hospital’s senior management team. The CFO is to manage the financial affairs of the hospital according to the business unit goals, sound business practices, corporate and hospital’s policies and procedures according to the government regulatory requirements. The incumbent will ensure efficient utilization of resources in order to meet the needs of the organization and will maintain compliance with all federal, state, local, and government agencies and accrediting bodies as required. Participates in executing administrative decisions as these functions relate to and impact the provision of total patient care. Works collaboratively and cooperatively with all departments and with members of the Medical Staff to accomplish the strategic goals and to achieve and enhance the mission of Monterey Park Hospital. This position requires the full understanding and active participation in fulfilling the Mission of Monterey Park Hospital. It is expected that the Chief Financial Officer demonstrate behavior consistent with the Core Values. The Sterile Processing and Distribution Technician maintain equipment and supplies in a state of sterility to be used in special procedures, the Operating Room, and Labor & Delivery. Maintains medical and surgical asepsis and principles of Standard Precautions at all times. SPD Tech also maintains equipment inventory and disinfection for patient care equipment used in the hospital. Decontaminates / disinfects reusable instruments, supplies and equipment, wearing proper protective attire.
