The Certified Nurse Assistant, reporting to the Nursing Unit Director, with assignment and direction from the Registered Nurse &/or LVN. This position is directly responsible for the delivery of high quality individualized basic nursing care. The Nursing Assistant supports the hospital mission and vision. The Nurse Assistant utilizes knowledge of patient’s age, developmental status, spiritual and cultural diversity into the provision of patient care. The position contributes to the provision of safety and quality nursing care that results in positive outcomes in patient care.
EDUCATION, EXPERIENCE, TRAINING
- Completion of Nursing Assistant Course Cert, Current certification for Nurse Aide (C.N.A.).
- High School Graduate or equivalent preferred .
- Six months acute hospital experience preferred.
- Current Basic Life Support/AHA .
Under the general supervision of the case management Director acts as a patient advocate/case manager to hospital clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Looks for opportunities to reduce cost while ensuring the highest quality of care is maintained. Applies review criteria to determine medical necessity for admission and continued stay. Provides clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost-effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care. Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care.

The Couplet Care Registered Nurse provides nursing care, treatment, counseling and health education to the OB couplet, i.e. postpartum mothers and their newborns, & to gynecologic patients. The Couplet Care RN utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care and educational needs of the patient/family/significant other to develop, implement, and revise the plan of care. The RN is responsible for planning and coordination of care across the continuum in an efficient and safe manner consistent with the unit's standard of care. This planning and coordination includes meeting and exceeding compliance with core measures, contributes to reduction in LOS, active participation with all aspects of quality, safety and performance improvement activities, accurate documentation of care, collaboration with all the members of the multidisciplinary team in providing outstanding customer service to patients, family members, physicians and the public. Serves as the Resource nurse when oriented and as directed. Supports the organization’s vision and mission. Utilizes knowledge of patient's age, developmental status, spiritual and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through evidence-based practice and performance improvement techniques that demonstrate positive outcomes in patient care. Under general supervision and medical direction, provides any or all of the designated respiratory care therapies designed to care for patients with deficiencies and abnormalities, which affect the pulmonary system and associates aspects of cardiopulmonary function.
