This position is responsible for providing patient care in a safe and effective manner. Responsibilities include directing PCA activity, synthesizing patient information, assessment of patients, planning of care/intervention, teaching of patients/family, implementation of treatment plan, MD orders, documentation and evaluation. Integration of patient care standards are incorporated into all clinical activities.

In collaboration with the Patient Experience staff, will perform purposeful daily rounding on patients and families in the facility to assess patient needs and collaborate with nursing and other staff to ensure that needs are met. The staff member will demonstrate professional courtesy at all times, andwill role-model excellent verbal, and non-verbal communication skills. The employee will respond to basic non-clinical concerns and provide appropriate referral for further assistance when needed.
- Completes daily rounds of inpatients and periodic rounds of outpatients/Emergency Department patients. Establishes rapport with patients and families; inquiries regarding satisfaction with care.
- Follows up on patient / family concerns, complaints and compliments. Advises Director/Manager/Supervisor for appropriate unit(s).
- Maintains the dignity and confidentiality of the patient; serves as a patient advocate.
- Maintains confidentiality & safe guards medical records and patient information in accordance with hospital department policies and HIPAA regulations.
- Seeks information or asks for assistance from others when necessary in order to assist patient/family.
- Responds in a timely manner to critical situations regarding patients, families, and visitors in a professional manner; provides effective communication of these incidents to appropriate personnel.

: Responsible for providing independent care in a variety of clinical settings to specified population of neurological patients. The stroke program coordinator coordinates the acute management of the stroke patient; clinical practice through direct staff and patient education; multidisciplinary collaboration. This position requires experience in acute/neurosurgical/or critical care nursing; collaboration with nursing management; demonstrated interest in working with the neurological patient population. The stroke program coordinator functions as an expert clinician, consultant, educator and leader in program specific patient care management ensuring measured outcome.
EDUCATION, EXPERIENCE, TRAINING
- Graduate of an accredited school of professional nursing certificate. Licensure in the state of California as a Registered Nurse (RN).
- Registered Nurse/ Nurse Practitioner in California
- Current BCLS Card (AHA) and maintain.
- The successful candidate will be required to pass competencies for this position. Minimal Two years’ experience in direct patient care in Neuro or Neurosurgery area; One year experience in management.
