LVN Discharge Planner is responsible for monitoring the admissions, continued stay, and discharge of patients following pre-established admission criteria in collaboration with RN Case Managers, Admitting, Physicians as well as other interdisciplinary staff.   The Discharge Planner must (1) ensure compliance with patient’s plan of care, (2) participate in interdisciplinary care management rounds; and (3) collaborate between multidisciplinary healthcare team members, primary physician, community agencies, HMOs/PPOs, CCS, etc., whose services may be required and/or related to the care needs of the patient after hospital discharge.    Discharge Planner must monitor individual nursing and medical plans of care/discharge plans and provide appropriate interventions to assure care is appropriate and coordinated.  Ensuring an efficient and appropriate discharge through education, consultation, and counseling with patients as needed, will be a crucial responsibility.    In addition, the discharge planner should collaborate effectively with RN Case Managers to ensure efficient discharge planning and assessment by communicating the appropriate discharge information and instructions to the primary care giver and primary physician and/or follow-up care agency.   Lastly, the discharge planner must ensure patients are transferred to appropriate approved facilities when required while preventing inpatient re-admissions.   This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support San Gabriel Valley Medical Center’s strategic plan and the goals and direction of the Performance Improvement Plan (PIP).   - Collaborate effectively with RN Case Managers and other interdisciplinary staff to assess patient’s individual discharge needs, establish, implement, monitor and evaluate patient centered discharge planning, provides appropriate interventions to meet patient’s health need and enhance the quality of patient care experience. Maximize patient satisfaction and promote cost effectiveness outcome. - Ensure compliance with patient’s plan of care, participate interdisciplinary care management rounds as needed. Assure care is appropriate, and coordinated throughout the hospital stay. - Ensure collaboration between multidisciplinary healthcare team members, primary physician, community agencies, HMOs/PPOs, CCS, etc., whose services may be required and/or related to the care needs of the patient after hospital discharge.  - Collaborate with RN Case Managers in identifying inappropriate admissions/transfers. Refer to Physician Advisor as needed. - Collaborate with RN Case Managers and Physician advisors to ensure patient care was provided appropriately and effectively, identify potential quality issues and delays in service. Address avoidable patient days effectively through education, consultation, and counseling as needed.  - Assure patients are transferred to appropriate approved facilities when required while preventing inpatient re-admissions. - Assist patients/family in making informed decisions by acting as their advocate regarding their clinical status and treatment options. - Develop effective working relations and cooperate with medical team throughout the entire case management process. - Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction - Record cases information, complete accurately all necessary forms and produce statistical reports as required. - Collaborate effectively with RN Case Managers to develop and update training and educational materials and present to appropriate staff, members and families. Facilitate patients' return to normal daily activities by teaching and making appropriate referrals for outside services/continued care.

Under the direction of Nursing Office Personnel (Nursing Director/Manager, Assistant Director of Nursing, nursing support manager or staffing coordinator) responds to and facilitates transport of patients throughout the organization. The definition of transport is described as, but not limited to, movement of patients to and from diagnostic testing/procedures, transfer of patients between floors/unit of care, transportation of remains to the morgue, and transport of goods/medical equipment throughout the organization. This position requires frequent walking and movement of patients/objects whose weight may be in excess of 25lbs.
