JOB SUMMARY:     Under the direct supervision of the supervisor, collects all birth certificate information from parents, completes and registers all live births in accordance to rules and regulations set by the County of Los Angeles Health Department. The Health Information Clerk II assists physicians and other medical personnel in achieving timely, accurate, and complete documentation practices.  Clerk II analyzes the assembled record for chart completion deficiencies, in accordance with regulatory requirements and Medical Staff Rules and Regulations.  Clerk II assigns deficiencies to the appropriate medical staff member, and data is entered for tracking and monitoring purposes.  Medical Staff members are notified, and held accountable for these chart completion items.  The Clerk II will assist the medical staff member in their chart completion effort, but will carry out the suspension process if the medical staff member does not complete their records in a timely manner.  Once the medical staff member addresses their documentation deficiencies or upon addition of a document to the analyzed record, the Clerk II will review this documentation and update or clear the deficiencies as appropriate.  The Clerk II is also responsible for answering telephone calls and assisting visitors (including but not limited to physicians, patients, and their family members), while maintaining confidentiality.  The Clerk II will further support the department needs through assistance in any of the Clerk I, Release of Information, or other duties as assigned. . Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation and evaluation, provided to the women and newborns during the antepartum, intrapartum, postpartum and neonatal stages of the childbirth process.  The L&D RN is responsible for directing and coordinating all nursing care based on established clinical nursing care in the Labor and Delivery Department setting, including, but not limited to, providing labor support and education, monitoring maternal & fetal conditions, and monitoring the mother`s and baby`s vital signs during labor & birth, and managing a variety of potential issues that may arise. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Serves as the Resource nurse role when oriented and as directed. Supports the organization’s vision and mission. Utilizes knowledge of patient's age, developmental status, spiritual and cultural diversity into the provision of patient care.  Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.    The Labor and Delivery RN provides direct care, communicates with physicians / Nurse Director / Co – workers, as appropriate about changes in the patient’s clinical situation including results of diagnostic studies and is able to respond quickly and accurately to changes in situation or response to treatment.   EDUCATION, EXPERIENCE, TRAINING Graduate of an accredited school of professional nursing certificate. Licensure in the state of California as a Registered Nurse (RN). Proficient in oral and written communication skills. Ability to practice leadership, interpersonal skills, communication skills, teamwork, quality improvement and excellent customer service, and professionalism. Current California RN license upon hire and maintained.             Required Experience: •minimum successful 1 year Couplet care or L&D experience • ­             Required Education: Graduate of approved nursing program certificate             BCLS, ACLS, NRP (AHA) upon hire. Basic Fetal Monitoring certificate upon hire, if no current Intermediate/ Advanced Fetal Monitoring course.             Other certificates: Birth & Beyond, and Intermediate/Advanced Fetal monitoring, within 6 months of hire, ­Proficiency in both written and oral English Other languages Spanish, Chinese (Mandarin/Cantonese)                 preferred.

The Charge Nurse is responsible and accountable for the quality of staff and functions during their shift. Directly supervises and communications with all staff along with the Director of Medical/Surgical.  Assists the Department   Director in the daily operation of the unit, directing, supervising and evaluating personnel, ensuring the proper use of time, equipment and staff.  Ensure the efficient functions of the department.  Performs direct patient care, only as needed, within the            scope of practice.  Continually assess care and direct staff to make immediate changes in the plan of care.   EDUCATION, EXPERIENCE, TRAINING   - Current and valid state RN License - Current BCLS, ACLS (AHA) upon hire and maintain current. - Current Basic Dysrhythmia - Bachelor Degree in Nursing (BSN) preferred.
