Manages and ensures the day to day operation of the following departments: Operating Room, Scheduling, Post-Anesthesia Care Unit. Provides nursing care and role models professional nursing practice in accordance with San Gabriel Valley Medical Center standards and standards of The Joint Commission. Maintains shift line management and responsibility in administration of personnel, fiscal, and material resources. Creatively responds to changes in the work setting. Participates in program planning for new projects while maximizing the throughput of the perioperative service line; to include preoperative, intraoperative and postoperative clinical areas. Knowledgeable in clinical areas of supervision, using defined processes to establish goals and outcomes, hich reflect systems integrations in today’s dynamic health care environment.
The Quality Clinical Abstraction will be responsible for the abstraction and assessment, measurement and reporting of The Joint Commission (TJC) Core Measures, CHART and other assigned clinical data abstraction reported internally or to external agencies. The Program Manager for Clinical Abstraction will utilize, understand, and apply specific clinical data definitions and abstraction rules associated with the management of the external benchmarking projects. He/She will act as the primary resource to the Quality Services Department and Clinical Focus Group Performance Improvement leaders and teams for any data abstraction guideline interpretation questions and/or issues related to CMS/TJC projects. He/she will insure the accuracy, integrity and validity of all data reported for internal and external benchmarking, including data transmitted and reported publicly on Qualitycheck.org (TJC), Calhospitalcompare.org (CHART), and Hospitalcompare.gov (CMS). He/she will serve as the Quality Department lead for CDAC Validation to ensure the hospital’s validation scores remain within the CDAC validation thresholds, and that requested charts are reviewed and sent in a timely manner to CDAC. He/she further insures that timely appeal of any cases which do not pass review. The Program Manager for Clinical Abstraction will also insure the integrity and validity of all related information that is used for analyzing services, practice patterns, and protocols of AHMC SGVMC. He/She will work collaboratively with others to promote the strategic goals and mission, vision and values of AHMC SGVMC.Qualifications:Graduate from an accredited school of nursing; Bachelor's Degree preferred.Current CA RN license.Minimum of five (5) years in performance improvement or case management.Knowledge of current CMS/Joint Commission definitions, abstraction rules and regulations.Ability to perform technical abstraction of patient records by abstracting pertinent information and preparing/presenting clinical information in such a manner as to highlight discrepancies in data. Ability to address multiple tasks that frequently have short timelines.Ability to maintain current and accurate databases and files.
Under the direction of a Radiologist, performs diagnostic ultrasound/vascular procedures. This position requires providing care/service to a critically through rehabilitating newborn through geriatric patient population in a manner that demonstrates an understanding of the functional/ developmental age of the individual served.

In conjunction with nursing and ancillary departments, coordinates and supports the assessing, planning, implementing, and evaluating of clinical education services for the Medical Center. Supports the management staff in assuring that staff development activities are consistent with current education standards and accreditation requirements. Plans, coordinates, implements, and evaluates patient education services as well as community health education and wellness programs.Assists nursing and ancillary department manages directors in identifying the learning needs of their respective staff.Develops and coordinates staff educational programs to meet the identified needs.Assures that staff education activities adhere to departmental policies and procedures.Evaluates the delivery of staff education activities in terms of positive outcome and quality improvement.Provides educational services in cost-effective manner.Serves as a resource to staff and supervisory personnel in meeting their educational needs.Assists other department in the development and implementation of mechanisms designed to test associate competency in job performance.Supports department safety related functions as needed.Supports management in assuring that staff development activities are consistent with current education standards and accreditation requirements.Assures that a sufficient number of regulatory mandated clinical educational activities are offered on an annual basis to hospital staff.Assures that all documentation of educational activities are complete, accurate, and comply with regulatory standard.Assists with the coordination and implementation of educational processes relative to validating the clinical competence of nursing and appropriate allied health staff.Coordinates the provision of BLS classes for the Medical Center; is responsible for all phases of the BLS program.Assists nursing and allied health managers in identifying learning needs of their patient population.Develops and coordinates patient educational programs to meet the needs identified.Evaluates the delivery of patient education services in terms of positive outcome and quality assurance.Assures patient education material and supplies are appropriate for age and cultural diversity of the patient population.Assists in the development and maintenance of a patient teaching documentation system consistent with current education standards and accreditation requirements.Develops and provides community education and wellness programs to meet the population needs.Conducts staff development in-services to improve the teaching skills of nursing and allied health personnel.Keeps abreast of the latest developments in patient education and networks with community agencies and other outside resources to insure high quality programs.Develops community and education classes to meet the learning needs of our population.Qualifications:Graduate of a School of Nursing; BSN required; Masters (preferred).Must have at least five (5) years medical/surgical or critical care background.Current CA RN License.Current BLS card.Current ACLS card.CPR Instructor certification within six months of hire.Adult teaching experience (required).Bilingual in Spanish, Vietnamese, or Chinese (preferred).
Facilitates, in collaboration with the Director of Quality & Risk Management accurate, timely and consistent clinical data to public reporting databases and agencies, including JCAHO (Oryx), and CMS. Reviewing paper and electronic medical records, interpreting, analyzing medical record documentation, and extrapolating pertinent health information needed for determining clinical quality and core measures required by The Joint Commission, CMS and other clinical reporting programs. Accurately entering the data directly into our electronic data collection database which is Quantros.
- Researching all available resources on missing, conflicting or ambiguous information in the medical record.
- Applying skilled judgment in determining that deviations from these guidelines may indicate an omission or inaccuracy within the patient’s medical records.
- Collects and trends physician compliance, sends out notifications when discrepancies are identified.
- Assists Infection Control with reviewing microbiology reports by interpreting and analyzing medical record documentation to determine if infection is community acquired or hospital acquired using CDC/NHSN surveillance definition.
Under the guidance and oversight of the Sr. Director of Quality, Risk and Infection Prevention and the Infection Prevention Committee, this position is responsible for monitoring, controlling and reducing nosocomial infections. The Infection Control Preventionist is also responsible for planning, implementing and evaluating infection control and prevention within the hospital.

The Speech Therapy Manager is responsible for planning, directing, implementing, coordinating and improving the activities of all Respective Services (Physical, Occupation, Speech, and Outpatient Rehabilitation). This includes staffing and program development in Acute Care, Acute Rehabilitation, and Outpatient Services, at both GMC and MON. The position reports to the Director of Rehabilitation Services at each GMC.Current CA SLP License and BCLS (AHA) certification required. Minimum of five (5) years experience which include acute care, outpatient and in patient rehab. Must have Supervisory experience and is able to teach, train and mentor staff.

Professional services to be provided to meet the psychosocial needs of the patient may include, but are not limited to, psychosocial assessments, brief treatment/counseling, crisis interventions, case consultation, patient education, resource management, assistance with discharge planning, and support groups.
The Social Worker is an integral part of the interdisciplinary team, providing mental health expertise in addition to community outreach, networking, and liaison with community programs as needed.
